Obesity and health risks

A study published in the Journal of the American Medical Association (JAMA) in 2004 shows how bariatric surgery can improve the following co-morbidities:

Type 2 diabetes was improved significantly in 86% of patients and resolved in 76% of patients.
High Blood Pressure was improved in 78% of patients and resolved in 61%.
High cholesterol levels were reduced in over 70% of patients.
Sleep apnea was eliminated 85% of patients.
The decision to have obesity surgery is not to be taken lightly, for us is very important to provide each patient with detailed information to determine whether they are a candidate for surgery and decide which bariatric procedure is right for them based on their individualized health history and lifestyle.

Central or Abdominal Obesity Carries Greater Health Risks

Abdominal obesity characterized by excessive visceral fat around the stomach and abdomen is one of the core symptoms of cardiovascular disease and insulin resistance syndrome.

Increased Health Risk of Premature Death


The risk of premature death rises with increasing weight. Individuals who are obese (BMI greater than 30) have a 50 to 100 percent increased risk of premature death from all causes, compared to individuals with a healthy weight.

Increased Health Risk of Heart Disease


The risk of heart attack, congestive heart failure, sudden cardiac death, angina or chest pain is increased in persons who are overweight or obese. High blood pressure is twice as common in adults who are obese than in those who are at a healthy weight. Obesity is associated with high triglycerides and decreased HDL cholesterol.

Increased Health Risk of Type 2 Diabetes


Over 80 percent of people with diabetes are overweight or obese. This may account for the newly invented word, “diabesity”®, which signifies the close association between obesity and diabetes.

Increased Health Risk of Cancers


Obesity is associated with an increased risk for some types of cancer including endometrial (cancer of the lining of the uterus), colon, gall bladder, prostate, kidney, and post-menopausal breast cancer.

Increased Health Risk of Fatty Liver Disease


The main cause of non alcoholic fatty liver disease is insulin resistance, a metabolic disorder in which cells become insensitive to the effect of insulin. One of the most common risk factors for insulin resistance is obesity, especially central abdominal obesity. The higher the BMI the worse the liver disease.

Increased Health Risk of Breathing Problems


Obstructive sleep apnea (that is, interrupted breathing during sleeping) is more common in obese persons.

Increased Health Risk of Arthritis


Musculoskeletal disorders, including osteoarthritis, are much more prevalent among obese patients, especially patients diagnosed with severe clinical or morbid obesity. Health studies show that obesity is a strong predictor for symptoms of osteoarthritis, especially in the knees. The risk of osteoarthritis increases with every 2-pound gain in weight.

Health Risks of Adult Obesity

There is now good evidence to show that adult obesity is associated with a wide range of health problems, these are summarised below.

Musculoskeletal system
  • Raised body weight puts strain on the body's joints, especially the knees, increasing the risk of osteoarthritis (degeneration of cartilage and underlying bone within a joint).
  • There is also an increased risk of low back pain.


Circulatory system
  • Raised BMI increases the risk of hypertension (high blood pressure), which is itself a risk factor for coronary heart disease and stroke and can contribute to other conditions such as renal failure.
  • The risk of coronary heart disease (including heart attacks and heart failure) and stroke are both substantially increased.
  • Risks of deep vein thrombosis and pulmonary embolism are also increased.


Metabolic and endocrine systems
  • The risk of Type 2 diabetes is substantially raised: it has been estimated that excess body fat underlies almost two-thirds of cases of diabetes in men and three quarters of cases in women. Diabetes currently affects nearly 200 million people worldwide and International Diabeted Federation predict that this will increase to over 330 million by 2025, with a massive burden in developing countries. Worldwide, the number of people with diabetes has tripled since 1985.
  • There is a greater risk of dyslipidemia (for example, high total cholesterol or high levels of triglycerides), which also contributes to the risk of circulatory disease by speeding up atherosclerosis (fatty changes to the linings of the arteries).
  • Metabolic syndrome is a combination of disorders including high blood glucose, high blood pressure and high cholesterol and triglyderide levels. It is more common in obese individuals and is associated with significant risks of coronary heart disease and Type 2 diabetes.


Cancers
  • The risk of several cancers is higher in obese people, including endometrial, breast and colon cancers.


Reproductive and urological problems
  • Obesity is associated with greater risk of stress incontinence in women.
  • Obese women are at greater risk of menstrual abnormalities, polycystic ovarian syndrome and infertility.
  • Obese men are at higher risk of erectile dysfunction.
  • Maternal obesity is associated with health risks for both the mother and the child during and after pregnancy. Click here for more information on maternal obesity


Respiratory problems
  • Overweight and obese people are at increased risk of sleep apnoea (interruptions to breathing while asleep) and other respiratory problems such as asthma.


Non-alcoholic fatty liver disease
  • The term ‘non-alcoholic fatty liver disease’ (NAFLD) refers to a range of conditions resulting from the accumulation of fat in cells inside the liver. It is one of the commonest forms of liver disease in the UK. If left untreated, it may progress to severe forms such as non-alcoholic steatohepatitis (NASH), fibrosis and cirrhosis. It has also been linked to liver cancer.
  • Obesity is an important risk factor for the condition: over 66% of overweight people, and over 90% of obese individuals, have NAFLD [1]. As levels of obesity have risen, so has the prevalence of NAFLD.
  • There is a lack of high quality data related to the prevalence of NAFLD in the UK, but international studies estimate that NAFLD is likely to affect around 20% of adults (estimates across countries range from 6% to 35%) and 10-15% of children worldwide [2, 3]. This is due to a number of factors including variations in diagnostic criteria, the invasive nature of diagnosis, and the lack of symptoms in people with mild forms of the condition.
  • Approaches to tackling the condition focus on weight reduction through a combination of diet and physical activity, but there is no specific evidence-based treatment for NAFLD.


Gastrointestinal disease
Obesity is associated with:
  • Increased risk of gastro-oesophageal reflux
  • Increased risk of gall stones
  • Psychological and social problems
  • Overweight and obese people may suffer from stress, low self-esteem, social disadvantage, depression and reduced libido.

Obesidad causante de la Apnea del Sueño


El dormir puede representar en el paciente obeso, un período de estrés, el cual puede tener un impacto negativo sobre la vida diurna del paciente y en algunos casos, puede relacionarse con la muerte. Los trastornos del dormir más comunes en el obeso son las alteraciones en la respiración durante el sueño como la llamada apnea del sueño que es una afección en la que se suspende temporalmente la respiración durante el sueño, siendo el primer obstáculo para un sueño reparador.

En general, los hombres con una circunferencia de cuello de 43 cm o más y las mujeres con circunferencia de cuello de 40 cm o más, poseen mayor riesgo de presentar apnea del sueño.
Los individuos con esta afección tienen disminución de la calidad del sueño ya que el organismo supera el problema con el despertar repetido y el reposo se ve interrumpido a veces por numerosos micro despertares que les impiden profundizar el sueño y descansar. La presentación más clásica de la apnea del sueño obstructiva es la aparición de ronquido justo después de dormirse.

La obesidad es origen de múltiples trastornos sistémicos de importante morbi – mortalidad, entre los que se destaca la apnea del sueño, pudiendo prevenirse o curarse con un buen control del peso
